Community Health Worker at the Library

TCCAP’s Community Health Workers can support community members with:
*Hands‑on assistance with applications, healthcare navigation
*Supporting appointment scheduling with primary care providers, IHHS providers, specialists, or behavioral health
*Explaining medical instructions, diagnoses, or treatment plans in plain language
*Connecting individuals to transportation services for medical appointments
*Help individuals connect to resources that support their health, stability, and overall well‑being
