Community Health Worker at the Orland Library

TCCAP’s Community Health Worker hours. They can support community members with:
- Hands‑on assistance with applications, healthcare navigation
- Supporting appointment scheduling with primary care providers, IHHS providers, specialists, or behavioral health services
- Explaining medical instructions, diagnoses, or treatment plans in plain language
- Connecting individuals to transportation services for medical appointments
- Help individuals connect to resources that support their health, stability, and overall well‑being
